Hospice Admissions Nurse (RN) Tucson, AZ US Responsibilities of the Hospice Admissions Nurse (RN) Coordinate hospice admissions based on the criteria set out by company guidelines. Reviews patient referrals, collaborates hospice care options with patients and family members, and plans with both the patient and the facility to prepare for hospice care. Work closely with the hospice marketer, potential hospice candidates, families and their corresponding medical team to ensure the transition to hospice is handled delicately. Communicates with other medical entities and heir teams, who may be potential referral sources, and as an advocate for the family or loved one choosing the end of life path through hospice. Complete hospice admission and evaluate the effectiveness of hospice services to the patient and family ongoing Adhere to Federal, state and accreditation requirements including Medicare and Medicaid regulations Maintain an ongoing knowledge of current drug therapies Referral of goals/weekly-informational, presentations and follow-up of past referrals Requirements of Hospice Admissions Nurse (RN) Bachelor or Master's in Registered Nursing preferred Must be licensed in the State of Arizona as a Registered Nurse One year of hospice RN experience Must be able to communicate verbally and written Must be a licensed driver with an insured automobile in good working order Should be skillful in organization and in the principals of time management and have knowledge of hospice admission and referral process The ability to access patient's homes which may not be routinely wheelchair accessible is required. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care Walking, sitting, stooping and standing and minimal to maximum lifting of patient's and turning pf patient's.
