Key Responsibilities

  • Build strong direct relationships with patients while they are receiving treatments or recovering in home or infusion suite.
  • Provide infusion nursing care and educate patients at the start of care in accordance with the physician's plan of treatment and company policies and procedures.
  • Train patients and/or caregivers in procedures and equipment. Provide direct patient care as required, i.e. peripheral IV starts, lab draws, dressing changes.
  • Monitor compliance, competence, and tolerance to prescribed therapy, interventions, and progress.
  • Provide other healthcare professionals with feedback on patients and gives suggestions for changes in procedure/protocol.
  • Maintain and update, per policy, clinical documentation which includes, but is not limited to; daily visit logs, AOB's, delivery tickets, on-call logs, assessments, subsequent notes, care plans, medication profiles, plan of treatment, case conference notes, progress notes, discharge summary and any other paperwork required by the company.
  • Document patient care activity and communication with the patient, physician, and pharmacist.
  • Participate in on call rotation as indicated by staffing and business needs.
  • Must maintain current and be in good standing with all state, federal and company set requirements to fulfill duties and responsibilities of the assigned position.Additional Responsibilities
    • Ability to establish and maintain positive clinical rapport with all patients and their families, representing AleraCare including working directly with patients and their families, maintain contact and clinically monitor all patients providing direct infusion therapy services.
    • Infuse medications on a regular basis in infusion suite or patient's home setting including monitoring and tracking vital signs, performing procedures such as IV placement, phlebotomy, and medication administration via peripheral infusion device; or PICC line; Implantable port; and/or Central Venous Catheters and as ordered by a licensed physician or practitioner.
    • Assess patient in home environments, including clinical assessment safety of environment, pain assessment, monitor effectiveness of medication treatment, develop plan of care, education for patients and patient's family members, including providing care.
    • Establish and maintain all clinical documentation including Plans of Care/Treatment (POC/POT), Clinical Summaries, medication changes and updates for AleraCare.

    • Willingness to work with most ages and diagnosis of patients receiving infusion therapy medication.
    • Clinical oversight of all patient functions provided by referring physician.
    • Collaborative relationships involving all agencies delivering services on behalf of/or in conjunction with AleraCare, including all nursing agencies in a contracted relationship, patient, families, significant caregivers, and other healthcare team members.
    • Maintaining current State Nursing licensure, as required by the State Department of Health, is mandatory.
    • Assist in securing and maintaining all licensure and accreditations warranted by AleraCare, including required ACHC, JCAHO, and any other licensure or credentialing, as necessary.
    • Compliance and adherence to all State and Federal regulations and laws, JCAHO, OSHA, HIPAA, CPR, all infusion pharmacy and nursing licensure accepted standards of practice.
    • Maintain all mandatory education required upon hire and annually per policy.
    • Function as clinical educator and resource.
    • May work in office or alternate settings including home settings.
    • Ability to travel on occasion.
    • Excellent driving record.
    • Maintains automobile liability insurance.
    • May be required to participate in after hours on call responsibilities or to work any day or shift as required to accommodate patient schedules in administering medications.
    • May be required to lift or carry equipment and supplies to patients' home.Qualifications
      • Registered Nurse with current license in Arizona.
      • Minimum three years nursing experience with a minimum of one-year infusion/IV experience preferred.
      • Must have and maintain current CPR certification.
      • Must possess a valid and current driver's license and auto insurance.
      • Advanced user computer skills with excellent customer service skills (written and verbal) a must.AleraCare supports a drug free work environment.Job Types: Part-time, PRNPay: From $40.00 per hourBenefits:
