Job Description:
The Nurse Practitioner will provide medical care for a variety
of the Pascua Yaqui Tribe's health units, which may include but not
be limited to wound care, preventative treatments for podiatry
patients, same day appointments for patients with minor illnesses,
clinical consultative, and evaluative and medical services for
clients receiving mental health services. The incumbent's duties
include medication management, counseling, and case management
services to clients. Deliver comprehensive, high quality medical
care in the outpatient setting, along with home visits, clinical
visits and facility visit with an emphasis on ongoing and
preventative health care. Deliver high quality care to a wide
variety of patient types including the spectrum between newborns to
the adult patient, depending upon specialty training. Evaluate need
for medications, medication management, medical supplies, and/or
ongoing patient education. Manage general medical problems. Conduct
duties associated with medical practice, including required forms
and documentation. Participate in an ongoing basis in a Continuing
Medical Education program. Attend committee meetings as assigned.
Staff the evening or weekend clinics as determined by Medical
Director. Maintain minimum patient productivity. Monitor and track
health care needs of assigned patients; work with other medical
providers to coordinate proper and effective care. Coordinate
referrals and follow up services with other Tribal departments;
respond to patient and staff inquiries, research possible community
resources, and ensure proper forms and documents are completed.
Enter and maintain clinical information and clinical notes into an
electronic charting system, in accordance with the Pascua Yaqui
Health Department Policies and Procedures. Work cooperatively and
directly with nursing staff according to their normally assigned
duties. Assure current DEA Certificate, renewed as required by law.
Provide patient care through home visits, clinical visits, and
evaluate need for medical supplies and/or ongoing patient
education. Perform other duties of a similar nature or level as
requested by supervisor or director. Knowledge of: Best practices
in healthcare; Medication management for clients; ICD-10
Classification System; Treatment planning; Tribal, state, and
federal regulations regarding healthcare maintenance; Medical
records and clinical documentation standards; HIPAA requirements of
Health Department; Yaqui culture, customs, resources and traditions
and/or a willingness to learn. Skills and Abilities: Clinical work
with health clients; Data collection and service tracking; Cultural
competency; work with diverse populations; Clearly document medical
records in an electronic format; Work independently, as well as
part of a team; Work in a fast-paced work environment; Establish
and maintain an effective working relationship with colleagues,
clients, and outside programs; Interact with other professionals in
the healthcare field; Understand and protect client
confidentiality; Submit reports in a timely and accurate manner;
Maintain up-to-date and accurate records and logs; Handle multiple
tasks and meet deadlines; Interact and maintain good working
relationships with individuals of varying social and cultural
backgrounds; Operate a variety of office equipment, including a
computer and related software applications; Good communication and
interpersonal skills as applied to interaction with co-workers,
supervisor, management, Council members, and the public. Have
ability to sufficiently exchange or convey information and receive
verbal and written work instructions. Licensed Nurse Practitioner
in the State of Arizona plus two (2) years of experience in an
ambulatory care setting preferred. Special Requirements Must be a
Family Nurse Practitioner licensed by the State of Arizona Board of
Nursing. Failure to maintain a current License will result in
removal from this position; Must possess and maintain a valid
Arizona Driver's License; Must have a current Level 1 Arizona
Clearance Card. Failure to maintain a current Level 1 Clearance
Card will result in termination.
