Our nurses are simply the best! Aventas Home Health is currently hiring experienced RN's for intermittent home health care.We provide flexible PRN visit schedules. This position responsibilities include: Oasis start of cares including in home client assessments, reviewing patient medical history and developing personalized care plans that support quality measures. Teach & educate patients to maximize their goals and outcomes. Observe & report signs, symptoms and changes in condition with physicians. Case management and discharge planning.
