classes to name a few!Job SummaryNurse-Family Partnership (NFP) is
a national home visiting program that collaborates with Casa de los
Ni--os and Easterseals Blake Foundation. The NFP nurse home visitor
is responsible for providing comprehensive community health nursing
services to women and their families eligible for the NFP program.
NFP nurse home visitor (NHV) is responsible for providing
comprehensive community health nursing services to women and their
families eligible for the NFP program, an evidence-based program
that has demonstrated significant critical outcomes for at-risk
women and their children and. The NHV builds and models a
strength-based, client-centered culture. The NHV is responsible for
maintaining the highest standards in clinical nursing practice and
adherence to the NFP model and the policies, procedures,
guidelines, and standards of NFP and the lead agency.Essential
DutiesProvides home visits to women and their families eligible for
the NFP program.Adheres to the NFP home visiting model.Maintains a
caseload of 25 clients.Supports policies, procedures, guidelines,
and standards of NFHVP and the lead agency.Participates in
community coalition in support of NFP.Promotes public awareness of
NFP.Develops and maintains community relationships to support
client referrals.Works a flexible schedule, including some evenings
and weekends.Meets training and agency requirements for the
position.Develops therapeutic relationships with clients and their
families in a home visiting environment.Performs home visiting in
accordance with the NFP model and guidelines.Assesses physical,
emotional, social, and environmental needs of clients and their
families related to the NFP domains.Assists clients and their
families in establishing goals and outcomes.Provides education,
support, and referral resources to clients.Consults and
collaborates with other professionals in providing services to
clients and their families.Engages in skill-building to meet NFP
NHV competency requirements.Meets with nurse supervisor for weekly
reflective supervision.Uses reflective practice in supervisory
sessions.Schedules joint home visits with nurse supervisor
quarterly.Participates in bi-weekly staff meetings and case
conferences.Submits NFP data to support staff for timely and
accurate input to the data collection system.Cooperates in review
and analysis of data collection system reports.Participates in
quality improvement efforts.Maintains confidentiality.Documents
appropriately.Performs related duties as assigned or required.NHV
has authority to make decisions regarding families on his/her
caseload within the scope of the job responsibilities.NHV must seek
supervision regarding these decisions; NHV must consult the nurse
supervisor before making decisions regarding reports to CPS or
police (unless immediate and imminent danger exists).
Requirements:
Bachelor of Science in Nursing.Current RN license (in good
standing).Proof of COVID immunization.Current CPR/BLS.Two years
recent experience in maternal/child health, public health, home
visiting, or mental/behavioral nursing preferred.Home visiting
experience preferred.Proficient written and verbal communication
skills and computer skillsMust be able to meet training and agency
requirements for the position.Must foster an environment that is
