Overview:

We are seeking a dedicated Physical Therapist to join our team. The ideal candidate will have a passion for patient care and possess a strong knowledge of physiology. Provide skilled therapy intervention and treatment to patients in a home setting. Including, appropriate utilization of treatment plans and assessment skills. Monitor carefully the patient status and progress toward goals as well as compliance with physician's orders. Responsible for educating patients and caregivers while preparing the patient/caregiver for discharge as appropriate.

Responsibilities:

- Provide physical therapy services to patients based on individual needs and treatment plans

- Conduct assessments to determine patients' physical condition and establish treatment goals

- Utilize medical terminology effectively in documentation and communication

- Implement therapeutic exercises and techniques such as trigger point therapy

- Collaborate with healthcare professionals to ensure comprehensive patient care

- Offer guidance on home community care for continued progress

Requirements:

- Degree in Physical Therapy from an accredited program

- State licensure as a Physical Therapist

- Strong understanding of physiology and anatomy

- Proficiency in trigger point therapy techniques

- Knowledge of geriatric care principles

- Excellent communication skills and ability to work within a multidisciplinary team

Job Type: Full-time

Pay: $85,000.00 - $95,000.00 per year
