Job Summary:We are seeking a skilled Registered Nurse to join our team as a Patient Navigator. In this role, you will be responsible for providing exceptional patient care and support to our patients and their families.Key Responsibilities:

• Assess patient needs and develop treatment plans in collaboration with physicians.
• Provide patient education and instructions on medication, procedures, and test results.
• Triage patients and provide immediate support as needed.
• Assist with patient charting and documentation.
• Compile and analyze medical data to prepare reports for outcomes measurement.
• Ensure accurate and timely completion of patient care protocols and procedures.Requirements:
  • Current State RN nursing license.
  • Current CPR certification.
