Continuum Rehab Group is seeking a dedicated and dynamic Outpatient Physical Therapist to join our compassionate team. In this role, you will provide exceptional care to patients across the continuum of care, including Assisted Living residents, while also leading our robust wellness program. This is an exciting opportunity to make a meaningful impact on the health and well-being of the community we serve.Conduct thorough evaluations, develop personalized treatment plans, and deliver high-quality outpatient physical therapy services across the continuum of care, with a primary focus on serving residents in Assisted Living and Independent Living settings.Qualifications: Graduate of an accredited Physical Therapy program. Licensed Physical Therapist in the state of Arizona (or eligibility for Arizona licensure). A minimum of 2 years of outpatient and long term care experience preferred. Demonstrated experience or a strong interest in developing and leading wellness programs. Excellent communication, leadership, and organizational skills.
