collaboration with Care Managers and Medical Providers as members

Opportunity: Part time FTE: 0.8 Job Exempt: No Work Shift: Days
(United States of America) Responsibilities: - Adheres to the LPN
scope of practice by contributing to the assessment of the health
status of individuals, families and communities. - Deliver direct
patient care, including administering treatments as prescribed,
assisting nurses and physicians with various procedures and
documenting individual patient needs, actions taken and patient
responses. - Administer prescribed medications, including
medications given through established parenteral/intravenous
infusions as prescribed by physician or standard protocols and
document according to policy. - Document and report patient
condition, care delivery and post procedure follow up. Assess
changes in patient s condition and report to nurse and physician as
needed. - Incorporates nursing activities that create a
compassionate, supportive and therapeutic environment for patients
and staff with the aim of promoting comfort and healing - Applies
Exceptional Patient Experience principles and incorporates into
care interactions - Other duties as assigned Qualifications: -
Minimum New graduate or experienced nurse from an accredited
practical nursing program - Licensed Practical Nurse NM - High
