(RN) provides direct care to residents and is a critical part of
the healthcare team. Working as part of a team, you will assess,
plan, implement, and evaluate resident care plans to promote
optimal health, safety, and well-being. Schedule: Part-Time,
Weekends, 8-10 hours per day Saturday and Sunday Primary
Responsibilities and Duties: Conduct comprehensive assessments of
residents’ health status, including vital signs, medical history,
and current symptoms. Collaborate with residents, families,
physicians, and other team members to establish goals of care and
develop appropriate care strategies. Regularly reassess resident
needs and adjust care plans accordingly, ensuring continuity and
responsiveness to changing conditions. Provide skilled nursing
interventions and treatments in accordance with physician orders
and established care protocols. Administer medications, perform
wound care, monitor vital signs, and manage medical equipment as
needed to support resident health and recovery. Coordinate with
ancillary services, including therapy, dietary, and social services
to ensure comprehensive and integrated care delivery. Educate
residents and families on health conditions, treatment options,
medication management, and self-care strategies to promote
understanding and empowerment. Provide emotional support,
counseling, and advocacy to residents and families facing health
challenges, fostering a trust and therapeutic relationship. Promote
preventive health practices and wellness initiatives to optimize
resident health and quality of life. Document nursing assessments,
interventions, and outcomes in resident medical records.
Participate in care conferences and team meetings to discuss
residents’ progress goals, and any changes in condition or care
needs. Adhere to infection control protocols and maintain a clean,
safe, and hygienic environment for residents, staff and visitors.
Collaborate with other healthcare professionals, including
registered nurses, physicians, therapists, and social workers, to
optimize resident outcomes and promote team work. Perform all other
duties assigned. Requirements: Job Qualifications: Valid license as
a Registered Nurse (RN) in the state where work is performed. 1-2
years of nursing experience, preferably in a long-term care or
skilled nursing facility setting. Strong clinical skills and
knowledge of nursing principles, procedures, and best practices in
long-term care. Excellent communication and interpersonal skills,
with the ability to interact effectively with residents, families,
caregivers, and healthcare professionals. Demonstrated ability to
prioritize tasks, manage multiple responsibilities, and adapt to
changing priorities in a fast-paced environment. Understanding of
regulatory requirements governing nursing practices and skilled
nursing facilities. Ability to build rapport with patients through
a compassionate and caring demeanor. Physical Requirements: Work
performed in a medical setting due to the need to work directly
with residents. Must be able to sit, stand, bend, reach, talk,
hear, stoop, kneel, crouch, stretch, and twist. Stamina to stand
and move about the facility to perform frequent rounds and respond
to emergent situations as needed. Emotional resilience and ability
to cope with the physical and emotional demands of working with
residents who may have complex healthcare needs. Capability to lift
and transfer residents safely, with proper body mechanics and
assistance as needed. Required vision includes close, vision,
distance, peripheral, and the ability to adjust focus. Maintain
regular attendance and timeliness as set forth within the work
schedule designated by the department supervisor/manager. The job
requires accepting criticism and dealing calmly and effectively
with high-stress situations. The ability to use your abdominal and
lower back muscles to support part of the body repeatedly or
continuously over time without "giving out" or fatiguing. The
ability to keep your hand and arm steady while moving your arm or
while holding your arm and hand in one position.
