Complete screening, assessments and Intakes of eligible members. Act as the primary point of contact for all members, activities and services funded by the SABG. Facilitate meetings and follow up on recommendations, to ensure continuity of care and service delivery, and educate members about service options and delivery.Must be able to obtain and retain a Level 1 Fingerprint Clearance Card. Minimum Qualifications:Bachelors or master's degree, no experience necessary.Associate's degree plus two (2) years Behavioral Health work experience.High School Diploma or GED plus four (4) years of full-time Behavioral Health work experience. At time of job offer, submission of 60 month (five year) driving record from DMV will be required. These standards must be maintained throughout employment and up-to-date driving records are reviewed regularly for current employees.Applicants must be able to pass a pre-employment drug screen.This is a Safety Sensitive Position.
