Job Summary The Medical Scribe performs all clerical and information technology functions for a physician in a clinical setting. The primary responsibility of the Medical Scribe is to record details during a client's medical appointment. This includes client history, diagnoses, current symptoms or issues, medical orders, and recommendations and will be documented in the electronic health record. The Medical Scribe must be knowledgeable about HIPAA, regulatory compliance, and billing and coding to assimilate new processes and procedures. Essential Duties

  • Maintains confidential/sensitive information to comply with legal and ethical requirements for preparing medical documents.
  • Listens to complex medical information and summarizes in clear, complete, and concise fashion.
  • Checks for accuracy, mistakes, or inconsistencies in medical documentation to reduce errors and all addenda must be signed off by a physician.
  • Ensures that all clinical data, lab or other test results, the interpretation of the results by the physician are recorded accurately in the medical record.
  • Documents medical visits and procedures accurately and thoroughly as they are being performed by the physician, including but not limited to patient medical history and physical exam, procedures and treatments performed by healthcare professionals, including nurses, nurse practitioner and physician assistants.
  • Documents physician-dictated diagnoses, explanations of risks and benefits, prescriptions and instructions for patient or family members for self-care and follow-up accurately and thoroughly.
  • Identifies and locates lab work an studies to be reviewed and placed in charts.
  • Prepares referral/diagnosis/medication/miscellaneous letters as directed by the physician via dictation or summary of the medical record.
  • Ensures that emails and/or faxes are sent out daily to all physicians involved in a patient's care, with all copies of pertinent reports or tests attached.

    Requirements:
    • Must be 21 years of age.
    • GED or High School Diploma required.
    • 80 -100 words per minute touch typing required.
    • Certified Medical Scribe Professional (CMSP) and/or Medical Assistant Certificate preferred.
    • Two (2) years of experience in health care or a related field.
    • Proof of COVID immunization.
    • Strong computer skills.
    • Confident, articulate, and professional speaking abilities.
    • Excellent English composition skills required to generate professional, polished writing at a high rate of production. Handwriting must be clear and legible.
    • Ability to chart in real time provider assessments and evaluation.
    • Accurately document information in an electronic medical record.
    • Able to operate common office equipment.
    • Spanish Speaking preferred but not required.
    • Advanced knowledge of Microsoft Office Suite computer applications, aptitude for learning computer software applications like EHR.
    • Must be able to meet training and agency requirements for the position.

    • Must possess or be able to qualify for and maintain a valid Arizona department of public safety level one fingerprint clearance card.
