Job descriptionThe Patient Care Navigator works with and supports patients with associated medical, ancillary and community navigation of breast imaging services. The position ensures that patients receive the proper follow-up imaging and referrals following breast imaging procedures. The navigator serves as a patient advocate and assists with patient communication and collaborates with other diagnostic and treatment centers to ensure patients receive timely and appropriate care.Essential Functions and Responsibilities of the JobNavigates clinical recommendation and/or referrals based on the American College of Radiology (ACR) practice guidelines.Navigates the daily patient flow, ensures timely appointments, supports navigating patient care through treatment planning and coordinating referrals for treatment; demonstrates patient advocacy.Understands the principle of clinical, physical, and psychosocial needs related to breast health and can communicate this information to the patient and family as appropriate.Collaborates with the healthcare team to achieve quality care goals and data outcomes for the breast care programManages data entryAbility to interact in a positive and constructive mannerAbility to prioritize and multitaskQualifications/Work Experience:Two years' experience in patient navigation and coordinationProficient in EMR computer systems, software and data entryExcellent verbal and written communication skillsSpanish fluency is preferredEducation/Licensure/Certification:Breast Navigation Certification - PreferredWilling to train the right candidate no experience necessary Job Type: Full-timePay: Staring at $16.00 per hour
