Job SummaryWe are seeking a highly skilled Patient Advocate Representative to join our team at Conifer Revenue Cycle Solutions. As a Patient Advocate Representative, you will play a critical role in ensuring that patients receive the financial assistance they need to access quality healthcare.Key Responsibilities

• Conduct thorough interviews with patients to determine their eligibility for various government and non-government programs.
• Identify and pursue all potential sources of payment, including auto insurance, Workers' Compensation, commercial insurance, and private insurance.
• Obtain and complete Confidential Financial Statement forms and assist patients in applying for benefits.
• Develop and maintain strong relationships with patients, hospital staff, and community resources to ensure seamless care coordination.
• Travel to patient homes for skip tracing and document collection, as needed.Requirements
  • High school diploma or equivalent required; minimum 2 years of experience in social services, hospital admitting, or a related field.
  • Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
  • Working knowledge of federal, state, and county programs, as well as proficiency in Microsoft Office and Windows.
  • Bi-lingual preferred (Spanish).Work EnvironmentThis role requires the ability to work in a fast-paced hospital environment, with frequent travel to patient homes and community resources.
